MedWorm: EffexorThe Venlafaxine "Heart Revenge:" A Short Report A 40-year-old white woman with a history of venlafaxine abuse was admitted to our emergency department within 3 days after attempting suicide. On admission, blood pressure was 140/85 mm Hg with 70 beats per minute. The resting 12-lead ECG showed a significant ST-segment elevation in the anterolateral leads consistent with acute anterolateral myocardial infarction (anterolateral AMI). Noninvasive monitoring of blood oxygen saturation showed 97% while she was breathing room air. Laboratory findings showed the increase of troponin I up to 16 µg/L (normal range, 0-0.05 µg/L) and creatine kinase MB subunit levels up to 166 U/L (normal range, 0-25 U/L). An emergency coronary angiogram was normal. Venlafaxine is generally prescribed for the treatment of major depression, as well as, generalized... Attention deficit disorder is not just a childhood disorder. Adults can be diagnosed with attention deficit disorder (ADHD) as well. Sometimes this is a result of the person’s childhood ADHD progressing into adulthood (up to 70 percent of children will continue to struggle with ADHD as adults). Other times, an adult can either simply never been diagnosed as a child, or develop the disorder later on in life. Adult attention deficit disorder looks a lot like childhood attention deficit disorder. That shouldn’t come as a surprise, since the symptoms are largely the same. If you answer “Yes” to six or more of the below symptoms, you may have adult ADHD: Often fails to give close attention to details or makes careless mistakes in course work, work, or other activities ... Source: American Journal of Psychiatry Area: Evidence > Medicines Management > References > 2010 - March > 16 Various classes of antidepressant medications generally induce remission of major depressive disorder in only about one-third of patients. In a previous study using mirtazapine or paroxetine alone or in combination from treatment initiation, the rate of patients who remitted within a 6-week period was twice that of patients using either drug alone. In this double-blind study, the authors sought to produce evidence for the superiority of different combinations of antidepressant drugs from treatment initiation. Patients (n = 105) meeting DSM-IV criteria for major depressive disorder were randomly assigned to receive, from treatment initiation, either fluoxetine monotherapy (20 mg/da... Updated Date: Mar 15, 2010 EST (Source: DailyMed Drug Label Updates for the last seven days (since May 20, 2007 EST)) This study investigates duloxetine in the treatment of patients with BPD. Eighteen outpatients with a DSM-IV-TR diagnosis of BPD were treated with open-label duloxetine, 60 mg/day, for 12 weeks. Patients were assessed at baseline, week 4 and 12 with the CGI Severity item, the BPRS, the HAM-D, the HAM-A, the SOFAS, the BPD Severity Index (BPDSI) and the HSCL-90-Somatization Subscale (HSCL-90 SOM). Adverse effects were evaluated using the Dosage Record Treatment Emergent Symptom Scale. Statistics were performed with the analysis of variance. Significant P values were ≤0.05. Fourteen patients completed the study. Four patients (22.2%) discontinued treatment in the first 4 weeks because of non-compliance. A significant change was found for: BPRS, HAM-D, SOFAS, BPDSI total score and items &l... Conclusions The combination treatments were as well tolerated as fluoxetine monotherapy and more clinically effective. The study results, which add to a growing body of evidence, suggest that use of antidepressant combinations from treatment initiation may double the likelihood of remission compared with use of a single medication. Paxil May Raise Death Risk for Those Taking Tamoxefin Breast cancer patients who are taking Tamoxefin to prevent a recurrence of the disease should not take the antidepressant Paxil (paroxetine), a new study suggests. Women in the study who took both drugs together had a greater than average risk of dying from breast cancer. The study looked at medical records for 2,430 breast cancer patients who had been taking tamoxefin. About 1 out 4 of these women had also taken Paxil; and, on average, about 41% of the time that the women were taking tamoxefin they were also taking Paxil. The researchers found that there was about one extra death from breast cancer for every 20 women who had taken the two drugs together. The researchers believe that the risk of death from breast cancer was increased because Paxil interferes with the cancer-preventing a... Coping with Antidepressant Side Effects All types of antidepressants can cause some side effects. The most common problems are sleepiness, dry mouth, constipation, nausea and sexual problems. Some people react badly to antidepressants; in others side effects can be quite mild. Different drugs have different risks: SSRIs, or selective serotonin reuptake inhibitors, such as fluoxetine (Prozac) may cause you to feel sick or more anxious during the first couple of weeks. Some types of SSRI can cause indigestion, but this can usually be avoided by taking them with food. They may interfere with sexual function, and there have been reports of episodes of aggression, although these are rare. Side effects of SSRIs tend to become less obvious after the first few weeks, while the body adapts to the drug. The exception is sexual side effect... Acupuncture comparable to venlafaxine for vasomotor symptoms in women with breast cancer Source: JCO Area: News The most debilitating adverse effects of chemotherapy and hormone therapy for women with breast cancer are vasomotor symptoms (hot flushes, night sweats). This randomised controlled preliminary study evaluated the safety and efficacy of venlafaxine and acupuncture, which has been found to be effective in reducing vasomotor symptoms in menopausal women. A total of fifty women with stage 0-III pre- or postmenopausal breast cancer on hormone therapy with tamoxifen or arimidex, were randomised to twelve weeks of acupuncture (n = 25) or venlafaxine (n = 25) treatment. Patients in the control arm took venlafaxine 37.5 mg orally at night for 1 week, then 75 mg, if tolerated, at night for the remaining 11 weeks. Patients in the acupuncture arm received treatments... Conclusions The FKBP5 genotypes associated with suicidal events in this study have been reported by others to cause the greatest degree of glucocorticoid receptor subsensitivity. These results are consistent with those of other studies linking alterations in the hypothalamic-pituitary-adrenal axis with suicidal behavior. The small number of events and lack of a placebo condition make these results preliminary. Replication with a larger sample and a placebo condition is needed to assess whether these events are related to treatment. (Source: Movement Disorders) Abstract Monoamine neurotransmission is efficiently terminated through synaptic reuptake of released neurotransmitters by high-affinity Na+- and Cl−-dependent neuronal monoamine transporters of the SLC6A family located in the plasma membrane of presynaptic nerve terminals. Recently, a low-affinity, high-capacity Na+- and Cl−-independent plasma membrane monoamine transporter (PMAT) belonging to the SLC29 solute carrier family has been cloned. PMAT was shown to transport monoamine neurotransmitters as well as organic cations such as 1-phenyl-4-methyl-pyridinium (MPP+). Thus, the PMAT which is highly expressed in the human brain may be involved in the modulation of central monoaminergic neurotransmission and it may be a target for drugs used to treat depression and schizoph... Not only drug interactions, environmental factors, disease processes and ageing are factors in the inter-individual metabolic capacity variance but also genetic factors probably play an important role, as is illustrated in the case presented. Besides therapeutic drug monitoring, genotyping some important cytochrome P450 (CYP450) enzymes was of additional value in explaining why the patient developed severe adverse effects and, moreover, did not experience any therapeutical effect of venlafaxine. Results indicated that the patient was a poor metabolizer for CYP2D6, the most important phase I enzyme to metabolize venlafaxine. It plays an important role in the etiopathogenesis and pharmacotherapy of mental disorders, such as depression or schizophrenia. In recent years, studies have shown that cognitive processes, which are impaired in the course of mental disorders, significantly change BDNF levels in the brain. To review the literature on antidepressants for the treatment of hot flashes in women, we searched the PubMed, International Pharmaceutical Abstracts, and MEDLINE databases from inception through May 2009. All publication types that included human participants and that were published in English were eligible for review. These articles, relevant abstracts, and additional references were used to collect pertinent data. Al... [Idiopathic intracranial hypertension and pregnancy. A report of two cases.] Authors: Jacopin-Bruneau L, Gommier B, Pierre F, Boog G The authors describe two cases of idiopathic intracranial hypertension (IIH) during pregnancy. The first case was discovered in a 21-year-old primigravida at 15 weeks gestation and thereafter six evacuation lumbar punctures were necessary, and a treatment by acetazolamide, 1000mg/day, was prescribed until parturition. Labor was induced at 37 weeks for intrauterine growth retardation associated with oligohydramnios. The second patient, a 38-year-old tertigravida primipara presented clinical signs of IIH at 28 weeks gestation in the context of preterm labour after a suicide attempt by massive ingestion of venlafaxine. She was given an evacuation lumbar puncture and went into labour at 29 weeks, after tocolysis failure. The diagnosis... Venlafaxine and trazodone are 2 popular antidepressant agents. Alteration of liver enzyme levels has been reported as a side effect of these drugs at normal doses. Herein we have reported the case of a 48-year-old woman without any previous history of liver disease, who developed fulminant liver failure after 4 months of venlafaxine and trazodone therapy. She required liver transplantation, a procedure that was successful with full patient recovery. The first 5 years of follow-up were uneventful. This case documented that venlafaxine and trazodone at normal doses can produce severe liver toxicity. Liver tests shoul... A 32-year-old man, with a history of an episode of paroxysmal atrial fibrillation 3 years prior, came to our Department as an outpatient for a routine re-evaluation. This study was based on national databases of records about prescriptions, births and medical diagnoses. Some of the information in these databases may have been misrecorded or missed. It is possible that the newborns of those women known to be taking prescription drugs, may have been more thoroughly examined for defects at birth, which would tend to bias towards finding more defects in this group. However, the overall proportion of malformations found in exposed and unexposed children suggests that this is not the case. VENLAFAXINE HYDROCHLORIDEtablet, Film Coated, Extended Release [Upstate Pharma, LLC] Updated Date: Sep 24, 2009 EST (Source: DailyMed Drug Label Updates for the last seven days (since May 20, 2007 EST)) Brain creatine kinase activity is increased by chronic administration of paroxetine. Authors: Santos PM, Scaini G, Rezin GT, Benedet J, Rochi N, Jeremias GC, Carvalho-Silva M, Quevedo J, Streck EL Major depression is a serious and recurrent disorder often manifested with symptoms at the psychological, behavioral, and physiological levels. In addition, several works also suggest brain metabolism impairment as a mechanism underlying depression. Creatine kinase (CK) plays a central role in the metabolism of high-energy consuming tissues such as brain, where it functions as an effective buffering system of cellular ATP levels. Considering that CK plays an important role in brain energy homeostasis and that some antidepressants may modulate energy metabolism, we decided to investigate CK activity from rat brain after chronic administration of paroxetine (selective serotonin... To sweat or not to sweat? A hypothesis on the effects of venlafaxine and SSRIs Summary: The hypothesis put forward here attempts to explain how the efficacy of venlafaxine against climacteric symptoms, including sweating, can be reconciled with the fact that this medication is known to cause sweating as an adverse side-effect. Peripherally, the sweating function is regulated noradrenergically by the sympathetic nervous system, central noradrenergic signal transmission being subject partly to inhibitory, partly to excitatory influences by serotonin (5-HT). Theoretically, sweating can be both initiated and inhibited by the activity of selective 5-HT reuptake inhibitors (SSRIs), so that the noradrenergic “tone” resulting from the interaction of noradrenergic and serotonergic neurons in the various regions of the brain probably determines the degree of sweating. Melatonin receptor agonist agomelatine: a new drug for treating unipolar depression. Authors: Bourin M, Prica C Agomelatine markedly differs from other classes of antidepressant drugs: its primary molecular targets in vivo are the melatonin MT(1) and MT(2) receptors, where it acts as a potent agonist, and the 5-HT(2C) receptors, where it exerts clear-cut antagonist properties. Agomelatine across a wide range of clinical trials suggests that agomelatine offers an important alternative for the treatment of depression, combining efficacy, even in the most severely depressed patients, with a favorable side-effect profile. AbstractManic switching during antidepressant treatment has been reported with every class of antidepressant drugs. Serotonin-noradrenaline reuptake inhibitors (SNRIs) have been increasingly used for the treatment of unipolar and bipolar depression and are well tolerated and sufficiently effective because of their dual mechanism of action. A case of duloxetine-induced hypomania in a non-bipolar patient is presented, and a brief review of all cases of SNRIs’ induced mania and hypomania has been carried out. The available data suggest that SNRIs, especially venlafaxine, can induce mood switching in patients with bipolar depression and in certain patients with unipolar depression, but the potential of duloxetine and milnacipran to induce manic or hypomanic symptoms cannot be disregarded... Regarding “Comprehensive Analysis of Remission (COMPARE) with Venlafaxine Versus SSRIs” Nemeroff et al. The authors show a small but significant advantage of venlafaxine over fluoxetine in achieving remission. They report, compared with all SSRIs, a number needed to treat (NNT) of 17, which would be of public health relevance. In a recent review () based on published studies with more rigorous and restrictive inclusion criteria, we could not show a statistically significant advantage of venlafaxine versus SSRIs in remission rates but found only a small advantage in response rates.
