  The Venlafaxine "Heart Revenge:" A Short Report

A 40-year-old white woman with a history of venlafaxine abuse was admitted to our emergency department within 3 days after attempting suicide. On admission, blood pressure was 140/85 mm Hg with 70 beats per minute. The resting 12-lead ECG showed a significant ST-segment elevation in the anterolateral leads consistent with acute anterolateral myocardial infarction (anterolateral AMI). Noninvasive monitoring of blood oxygen saturation showed 97% while she was breathing room air. Laboratory findings showed the increase of troponin I up to 16 µg/L (normal range, 0-0.05 µg/L) and creatine kinase MB subunit levels up to 166 U/L (normal range, 0-25 U/L).   Efficacy and tolerability of duloxetine in the treatment of patients with borderline personality disorder: a pilot study

This study investigates duloxetine in the treatment of patients with BPD. Eighteen outpatients with a DSM-IV-TR diagnosis of BPD were treated with open-label duloxetine, 60 mg/day, for 12 weeks. Patients were assessed at baseline, week 4 and 12 with the CGI Severity item, the BPRS, the HAM-D, the HAM-A, the SOFAS, the BPD Severity Index (BPDSI) and the HSCL-90-Somatization Subscale (HSCL-90 SOM). Breast cancer patients who are taking Tamoxefin to prevent a recurrence of the disease should not take the antidepressant Paxil (paroxetine), a new study suggests. Women in the study who took both drugs together had a greater than average risk of dying from breast cancer. The study looked at medical records for 2,430 breast cancer patients who had been taking tamoxefin. About 1 out 4 of these women had also taken Paxil; and, on average, about 41% of the time that the women were taking tamoxefin they were also taking Paxil. We report a cardiac complication in a patient treated with regular doses of venlafaxine. A 49-year-old man with prior normal cardiac function and stable chronic hepatitis C was treated for a major depressive disorder with usual doses of venlafaxine during an 8-month period until the occurrence of a cardiogenic shock in a context of dilated cardiomyopathy. Abstract: Although acute hepatitis may be a side effect of many medications, most cases are reversible after treatment interruption, and fulminant hepatic failure (FHF) is rare. AbstractWith respect to the pharmacological characteristic, venlafaxine is comparable with tricyclic antidepressants (TCAs), and venlafaxine might be comparable in efficacy. We performed a systematic review investigating the relative efficacy and tolerability of venlafaxine compared with TCAs (imipramine, clomipramine, amitriptyline, nortriptyline and desipramine). Relevant double-blind randomised trials were identified from systematic searches of electronic databases. An exact analysis of the estimated odds ratios of response of the TCA relative to venlafaxine showed no overall significance of treatment effect (P = 0.38).   Comparative study on the gastroprotective potential of some antidepressants in indomethacin-induced ulcer in rats.

This study investigated the antiulcer activities of tianeptine, trazodone, and venlafaxine on indomethacin-induced ulcers in rats; and evaluated tianeptine's effects on oxidant and antioxidant parameters in rat stomach tissue. The results show that trazodone and venlafaxine did not prevent indomethacin-induced ulcers. Tianeptine, however, decreased indomethacin-induced ulcers significantly at all doses used (6, 12, and 25 mg/kg). Famotidine, an H(2) receptor blocker, showed the highest antiulcer activity. We were of course aware of their meta-analysis, which focused on a subset of the published studies comparing venlafaxine and various selective serotonin reuptake inhibitors (SSRIs) and regret that their report was not published in time to be available for discussion in our article. With respect to the first point raised by Koesters et al., we agree that the differences between the results of their meta-analysis and ours are small. We also agree that publication bias can distort the results of meta-analyses; this is why we included all randomized double blind trials comparing venlafaxine and SSRIs in major depressive disorder conducted by Wyeth before 2007, whether published or not. The case report describes a 39-year-old woman with restless legs syndrome (RLS) and trichotillomania (TTM). She was treated with venlafaxine and clonazepam with partial remission of RLS and no response for TTM. When aripiprazole was added to the clonazepam both RLS and TTM fully remitted.
