Clinical examination of the vulva showed a tender nodule of the right labium minus. Histology confirmed Langerhans cell histiocytosis. Systemic involvement was excluded. Within 1 month the use of clobetasol propionate ointment led to resolution of both the patient's symptoms and the clinical appearance of the affected right labium minus. Histology showed epidermal spongiosis with subcorneal pustules and a dermal infiltrate with eosinophils and neutrophils. Lesions were treated with topical clobetasol propionate 0.05% for 10 days followed by topical tacrolimus daily until complete resolution, and then twice weekly for 1 year, without relapse. The response to topical corticosteroids and tacrolimus further support the close relationship with erosive pustular dermatosis of the scalp. We report a case of severe psoriatic nail dystrophy involving all 20 nails successfully treated by low dose MTX. A previously healthy 11-year-old girl presented with painful deformity involving all 20 nails that developed over a one-month period. Examination revealed geographic and fissured tongue, as well as severe nail dystrophy of all 20 nails characterized by erythematous swelling of the nail folds, yellowish discoloration of nail plates with pitting, severe crumbling and destruction, transverse depressions, prominent oil spots, and swelling of proximal nail. They describe a case in which a 28 year old woman used skin lightening creams for 7 years. She presented with weight gain (12.7kg in 3 years), inability to conceive successfully, recurrent vulval candidiasis, bruising, pigmented striae on her body and a round face. An initial diagnosis of Cushing’s syndrome was made, however, blood results found low levels of cortisol and corticotrophin. After enquiring further about drugs, the patient told the clinicians about the cream she had been applying.
