Source: BMJ Area: News The British Medical Journal has featured a review on the diagnosis and management of lichen sclerosis. The review starts with a case vignette, and discusses clinical features and investigations required for the diagnosis.   In terms of management, the review states, "After diagnosis, the accepted treatment is with the very potent topical steroid clobetasol propionate ointment. No controlled trials have been published to establish the best regime, but the guidelines suggest a single application at night for four weeks followed by alternate nights for four weeks and then twice a week for four weeks.   Rapid resolution of primary vulval adult Langerhans cell histiocytosis with very potent topical corticosteroids

We report a 62-year-old female patient presenting with an 18-month history of pruritus and burning of the vulva. Clinical examination of the vulva showed a tender nodule of the right labium minus. Histology confirmed Langerhans cell histiocytosis. Systemic involvement was excluded. Within 1 month the use of clobetasol propionate ointment led to resolution of both the patient's symptoms and the clinical appearance of the affected right labium minus.   Erosive pustular dermatosis of the leg: Long-term control with topical tacrolimus

We report the cases of two women who developed erythematous skin plaques with pustules that coalesced and evolved into erosions and crusted areas. Histology showed epidermal spongiosis with subcorneal pustules and a dermal infiltrate with eosinophils and neutrophils. Lesions were treated with topical clobetasol propionate 0.05% for 10 days followed by topical tacrolimus daily until complete resolution, and then twice weekly for 1 year, without relapse. The response to topical corticosteroids and tacrolimus further support the close relationship with erosive pustular dermatosis of the scalp.   Severe 20-nail psoriasis successfully treated by low dose methotrexate.

We report a case of severe psoriatic nail dystrophy involving all 20 nails successfully treated by low dose MTX. A previously healthy 11-year-old girl presented with painful deformity involving all 20 nails that developed over a one-month period. Examination revealed geographic and fissured tongue, as well as severe nail dystrophy of all 20 nails characterized by erythematous swelling of the nail folds, yellowish discoloration of nail plates with pitting, severe crumbling and destruction, transverse depressions, prominent oil spots, and swelling of proximal nail. The aim of the present study was to evaluate through ultrasound imaging the response to treatment of twice and once daily application of clobetasol propionate 0.05% foam on psoriatic skin as well as the atrophogenic potential of once daily application of the foam on healthy skin. The study included a total of 40 participants, 30 affected by Psoriasis vulgaris and 10 healthy volunteers.   Treatment of scalp psoriasis with clobetasol-17 propionate 0.05% shampoo: a study on daily clinical practice

Conclusions Short-contact treatment with clobetasol-17 propionate 0.05% shampoo has high user convenience and patient satisfaction rates. Moreover, the treatment is well-tolerated and efficacious from patients' perspective. Subgroup analyses did not reveal factors predicting treatment outcome, although treatment success tended to be more evident in patients who had received fewer treatments previously.   Effect of Intracanal Corticosteroids on Healing of Replanted Dog Teeth after Extended Dry Times

This study investigated the effect of potent intracanal corticosteroids on periodontal healing of replanted avulsed teeth and evaluated the systemic absorption of these corticosteroids. Sixty-seven extracted dog premolar roots were randomly assigned to one of the following groups: groups 1–3 filled with gutta-percha and replanted immediately and after 40 and 60 minutes, respectively; groups 4 and 5 filled with 0.05% clobetasol; and groups 6 and 7 filled with 0.05 % fluocinonide. Groups 4 and 6 were replanted after 40 minutes and groups 5 and 7 after 60 minutes. After 4 months, roots were evaluated histologically for signs of periodontal healing.   Melanocytic proliferations associated with lichen sclerosus in adolescence

In this report, we describe the case of a prepubertal girl presenting with a junctional melanocytic nevus of the labia minora superimposed by lichen sclerosus. The lesion was completely excised and free margins confirmed by histological examination. After a 3 months course of topical clobetasol treatment the girl remained asymptomatic. No local recurrence was notified with a follow-up time of 12 months. This study confirmed the incidence of mouth ulcers in renal transplant patients treated with sirolimus. The ulcers did not appear to be attributable to a dose-dependent mechanism, thus corroborating the results reported in the literature. However, a contributory role of transition from tacrolimus to sirolimus in their appearance was not seen in this study. Combination of sirolimus with mycophenolate mofetil appeared to have no bearing on the incidence of mouth ulcers. Clobetasol cream, whether or not given together with an anaesthetic solution, appears to reduce pain and shorten healing times, and could thus avoid discontinuation of treatment on account of mouth ulcers.   Treatment of erosive vulvovaginal lichen planus with methotrexate.

We report the successful treatment of severe long-standing erosive vulvovaginal lichen planus in four adult female patients using 2.5-7.5 mg of oral methotrexate once weekly in conjunction with topical clobetasol dipropionate 0.05% ointment and tacrolimus 0.03-0.10% ointment. All cases experienced improvement in symptoms and healing of lesions within 4-8 weeks. Methotrexate was well tolerated and no adverse events have been observed in any of the patients at follow up 4-6 months later.   Two Cases of Cushing's Syndrome Due to Overuse of Topical Steroid in the Diaper Area1

We report two cases with iatrogenic Cushing's syndrome with different clinical outcomes due to abuse of same potent topical steroid clobetasol propionate. One of them died because of fatal disseminated cytomegalovirus infection. The other patient recovered completely. Physicians and parents should be informed about the adverse effects of such potent topical corticosteroids and physicians should prescribe less potent agents, especially during infancy. Histopathologic examination of a biopsy specimen showed findings of interstitial granulomatous dermatitis. Clinical correlation suggested a diagnosis of interstitial granulomatous dermatitis with arthritis. She presented with weight gain (12.7kg in 3 years), inability to conceive successfully, recurrent vulval candidiasis, bruising, pigmented striae on her body and a round face.   Solid lipid nanoparticles prepared by solvent diffusion method in a nanoreactor system.

In this study, water-in-oil (W/O) miniemulsion was used as nanoreactor to prepare solid lipid nanoparticles (SLN) by solvent diffusion method. n-Hexane, Tween 80 and Span 80 were used as the oil phase and surfactant combination for preparation of W/O miniemulsion, respectively. The stable miniemulsion with the particle size of 27.1+/-7.6nm was obtained when the composition of water/Tween 80/Span 80/n-hexane was 1ml/18mg/200mg/10ml. Clobetasol propionate (CP) was used as a model drug. The physicochemical properties of the SLN, such as particle size, zeta potential, surface morphology, drug entrapment efficiency, drug loading capacity and in vitro drug release behaviors were investigated, comparing with those of SLN prepared by conventional aqueoethod.   Lichen striatus following solarium exposure.

Authors: Ciconte A, Bekhor P Two cases of lichen striatus occurring after solarium exposure are presented. Both patients were adult females who regularly attended a solarium prior to the development of their skin lesions. A 19-year-old woman attended the solarium twice weekly for 8 weeks prior to the appearance of lichen striatus on the chest and epigastrium. The condition resolved completely with the twice daily application of calcipotriol ointment for 6 months. The second patient, a 40-year-old woman, attended the solarium once weekly for 3 months before lichen striatus developed on the abdomen. She was treated with clobetasol propionate ointment, calcipotriol ointment and intralesional triamcinolone acetonide and the eruption cleared 7 months after onset. The histopathological pictu...

GlaxoSmithKline has announced that due to raw material supply and manufacturing issues related to the requirements of the production of Dermovate-NN cream, it is discontinuing the manufacture of this product. GSK will cease to supply the product from June 2007, and due to the manufacturing issues, it is likely that supply will be restricted to hospital pharmacy only until this time.   Clinical review of 202 patients with vulval lichen sclerosus: A possible association with psoriasis.

Authors: Simpkin S, Oakley A Two hundred and two patients with clinically typical or biopsy-confirmed vulval lichen sclerosus were reviewed either at consultation (75%) or by retrospectively examining their chart. At diagnosis, 79% were 50 years or older. Ninety-six per cent complained of itching, pain and/or dyspareunia. Lichen sclerosus most often affected the labia minora and perineum but 50% had perianal and 13% had extragenital disease. Thirty-five patients gave a history of psoriasis (17%), which affected the vulval area in 10. Thyroid disease was reported in 39 patients (19%), and 33 gave a family history of thyroid disease.   SMC accepts Clarelux (clobetasol cutaneous foam) for use in steroid-responsive dermatoses of the scalp

The Scottish Medicines Consortium (SMC) has advised that Clobetasol propionate 0.05% cutaneous foam (Clarelux®) is accepted for use within NHS Scotland for short-course treatment of steroid responsive dermatoses of the scalp such as psoriasis, which do not respond satisfactorily to less potent steroids. The Committee noted that Clarelux offered an alternative to other scalp applications of clobetasol propionate at a similar cost (depending on the rate of application). (Source: NeLM Headline News)
