Despite this, the lesion continued to grow. The patient had a history of peripheral vascular disease and had resumed receiving warfarin sodium for this 1 week before appearance of the lesion. A 60-year-old woman with squamous cell carcinoma of the glottis underwent laryngectomy, anterior neck dissection, and pectoralis flap reconstruction of the anterior esophagus. Postoperatively, she was started on clindamycin for surgical site infection prophylaxis. On postoperative day three, increased drainage from the surgical site was noted. On postoperative day six, the patient had to return to the OR for exploration, and the site revealed purulent material with involvement of the jugular sheath. A collection of 179 isolates of C. difficile obtained from symptomatic adult patients in southern Scotland between 1979 and 2004 was used to determine changes in the prevalence of epidemiological types and antibiotic susceptibilities to common antibiotics. In the report, Thiboutot et al present combined data from two randomized controlled trials (RCTs) as if the two RCTs were in fact one large RCT. Unfortunately, such an approach is invalid.
