The Food and Drug Administration has issued a warning to Eli Lilly & Co. and United Therapeutics that a Web page and two patient videos were in violation of the agency's promotional rules. Adcirca (tadalafil) is indicated for pulmonary arterial hypertension to improve exercise ability. The FDA cited the Web page for failing to include any contraindications, warnings, or precautions for the drug, which “misleadingly suggests that Adcirca is safer than has been demonstrated,” said the warning letter. The two patient videos “seriously misrepresent what is known about the efficacy of Adcirca,” said the FDA.   Sudden Sensorineural Hearing Loss Associated with Vardenafil

We describe a patient who developed unilateral sudden sensorineural hearing loss possibly related to the use of vardenafil for erectile dysfunction. To our knowledge, only one other case of hearing loss related to this drug class has been published. Our patient was a 57-year-old man who came to the emergency department with right-sided mild-to-moderate hearing loss in the 500-3000-Hz range, confirmed by audiogram, that occurred after ingestion of vardenafil. The patient was hospitalized 2 days later for administration of intravenous dexamethasone, followed by oral prednisone. He reported that his hearing had improved on the fourth hospital day and was discharged 3 days later, continuing to taper the prednisone on an outpatient basis. The FDA announced this week that Atlas Operations has initiated a recall of their dietary supplements marketed for “male enhancement”. Analysis revealed that some of the products contained sulfoaildenafil, a sildenafil analogue.  This drug has the potential to cause dangerous decreases in blood pressure, especially for patients taking certain other medications such as nitroglycerin.  According to a press release from the company, some of the products recalled include Staminil, Love Fuel, Erexxx, Finally on Demand, and Whatzup.  The take-home lesson for clinicians is that in addition to asking that middle-aged gentleman with chest pain if he takes Viagra, Cialis, or Levitra before giving nitroglycerine, it is important also to ask if he takes any natural or herbal or over-the...
