Injection of botulinum toxin type A (BOTOX) into trigger zone of trigeminal neuralgia as a means to control pain

This article illustrates a case of persistent trigeminal neuralgia in a medically compromised 65-year-old female who did not respond to pharmacotherapy. She had undergone several peripheral neurectomies as well as a failed right posterior fossa exploration that resulted in a cerebrospinal fluid leak. Persistent pain over the right external nasal area and right mental region was relieved for several hours after daily injections of bupivacaine. A trial of a single dose of 100 units of botulinum toxin type A (BOTOX) diluted in 2.5 mL saline was injected into the external nasal trigger zone (60 units) and to the mental nerve region (40 units). She achieved complete pain relief in the external nasal region for 5 months. Pain recurred and the site was again injected with 100 units of botulinum t...

Botulinum Toxin Type A for Chronic Migraine

Abstract  Chronic migraine (CM) is the leading cause of chronic daily headache, a common and debilitating headache syndrome. The management of CM patients is challenging, with only limited benefit from available oral preventive medications. Botulinum neurotoxin (BoNT) has been used extensively to treat disorders associated with increased muscle tone. More recent scientific data support an analgesic effect of the toxin. The pharmacokinetic and pharmacodynamic profiles of BoNT make it an appealing candidate for migraine prevention. Results from older clinical trials on the efficacy of the toxin in CM were inconclusive. However, recent trials using more stringent inclusion criteria have shown positive results, supporting the use of the toxin in some patients with this disorder...

Ultrasound-Guided Botox Injections of Salivary Glands in Children with Drooling

This study aimed to test the hypothesis that ultrasound-guided botox injection of the salivary glands in children is effective temporary palliation for excessive drooling. The study population consisted of 6 patients who underwent botox injection of salivary glands one or more times at a mid-Atlantic regional children's hospital. Parents were interviewed by telephone regarding the effects of the botox procedure on their child's drooling. The children who comprised our study population exhibited decreased drooling, less respiratory distress, and increased quality of life after the injections. Further research with a larger sample is warranted to support these findings. (Source: Journal of Radiology Nursing)

We present evidence of prolonged denervation following BoNT injection of the vastus lateralis (VL) muscle to correct quadriceps muscle imbalance in patients with chronic anterior knee pain. Needle electromyography data were obtained from 10 subjects who had received a single BoNT treatment 5 to 19 months earlier as part of a clinical trial. Insertional and spontaneous activity, recruitment, and motor unit action potentials were examined. Clear differences between the injected and non-injected VL muscles, which correlated with the time since injection, were identified in all subjects. All 10 subjects studied with needle EMG showed evidence of persisting denervation in the BoNT-A injected VL muscle beyond the period of neuromotor recovery expected from animal experimental studies. © 2010 Mo...

Four unrelated patients with lubs X-linked mental retardation syndrome and different Xq28 duplications

The Lubs X-linked mental retardation syndrome (MRXSL) is caused by small interstitial duplications at distal Xq28 including the MECP2 gene. Here we report on four novel male patients with MRXSL and different Xq28 duplications delineated by microarray-based chromosome analysis. All mothers were healthy carriers of the duplications. Consistent with an earlier report [Bauters et al. (2008); Genome Res 18: 847-858], the distal breakpoints of all four Xq28 duplications were located in regions containing low-copy repeats (LCRs; J, K, and L groups), which may facilitate chromosome breakage and reunion events. The proximal breakpoint regions did not contain known LCRs. Interestingly, we identified apparent recurrent breakage sites in the proximal and distal breakpoint regions. Two of the four pati...

Botulinum Toxin Type A for the Treatment of Glabellar Lines in Chinese: A Double-Blind, Randomized, Placebo-Controlled Study

This is the first well-controlled study of the use of botulinum toxin type A (BoNTA) for glabellar lines in China. To evaluate the safety and efficacy of BoNTA in the treatment of glabellar lines in Chinese subjects. A total of 227 subjects received a single treatment in a 3:1 randomization ratio of BoNTA (20 U):placebo and were observed for 120 days after injection. Effective outcome measures included investigator's rating of wrinkle severity at maximum frown and rest and subjects' global assessment and self-perception of age. A significantly higher responder rate at maximum frown, ranging from 94.1% at day 30 to 52.9% at day 120, was noted in the BoNTA group. The proportion of subjects with none or mild glabellar lines at rest was 66.7% in the BoNTA group at day 30. Most (95.3%) of the s...

Needle Preference in Patients Receiving Cosmetic Botulinum Toxin Type A

This study sought to determine whether patients receiving BTX-A have a preference for 30-gauge or 32-gauge needles based on the amount of pain and bruising experienced. Thirty-seven subjects received BTX-A on the right side of the face using a 30-gauge needle and on the left side using a 32-gauge needle. Subjects were masked to needle size. They were then asked to rate injection pain on an 11-point numerical rating scale and to note any bruising. Physician preference was also evaluated. There were no statistically significant differences in the amount of intra-procedural pain (p=.37) or the level of post-procedural pain and discomfort (p=.76) experienced. Twenty-seven percent of subjects reported greater bruising with the 32-gauge needle, versus 29.7% with the 30-gauge needle. The physicia...

Treatment success for overactive bladder with urinary urge incontinence refractory to oral antimuscarinics: a review of published evidence

Conclusions: A treatment success definition that encompasses the many components of underlying OAB with UUI symptoms is currently not practical as a consequence of difficulties in measuring urgency. The treatment success outcome of >=50% improvement in incontinence episodes was associated with a clinically meaningful improvement in disease-specific HRQoL for those with neurogenic OAB with UUI. The recommended success definition is less restrictive than a measure such as complete continence but includes patients who are satisfied with treatment and experience meaningful improvement in symptoms. A standardized measure of treatment success will be useful in clinical and health economic applications. (Source: BioMed Central)

Allergan, the manufacturer of Botox (onabotulinumtoxinA), has sued the Food and Drug Administration, claiming that the agency's restrictions on off-label promotions are inhibiting the company's ability to “proactively share truthful and relevant information with the medical community,” according to a company statement. FDA's constraint that Allergan discuss off-label uses for Botox only in general terms violates the first amendment and hurts patients, Allergan claimed. To fully inform physicians, the company said it needs to describe dosing guidelines, patient selection criteria, and proper injection technique. “We believe that the inability to share such important information proactively with the medical community violates the First Amendment and potentially diminishes the quality o...

Lower urinary tract dysfunction in MS: management in the community.

Authors: Panicker J, Haslam C Lower urinary tract dysfunction in multiple sclerosis is common and is highly amenable to treatment. Individuals may have bladder storage and/or voiding dysfunction. The risk of progression to renal failure is low and hence lower urinary tract dysfunction should be considered medically manageable in most individuals. Evaluation begins with history taking and is supplemented by using a bladder diary. Ultrasonography is used to assess the degree of incomplete bladder emptying, and for assessing upper urinary tract damage. Incomplete bladder emptying is most often managed by clean intermittent self-catheterization and should be initiated if post-void residual urine is greater than 100 mls. Storage symptoms are most often managed using antimuscarinic medicatio...

  What Is Dystonia?

Dystonia is a rare neurological movement disorder characterized by involuntary muscle contractions, which force certain parts of the body into abnormal, sometimes painful, movements or postures. It can be inherited due to genetic abnormalities as seen in multiple sclerosis. It can also be caused by brain injuries, toxins and some medications. Antipsychotic medication, often prescribed in the treatment of bipolar disorder, is one class of drugs that can cause dystonia, which is one of several extrapyramidal side effects. Generally it is the first generation antipsychotics also known as typical antipyschotics that can cause these side effects. There is no cure for dystonia. However, there are a few options for treating the symptoms of the disorder. One of the most common treatments is Boto...

We smile because we are happy, and we frown because we are sad. But does the causal arrow point in the other direction, too? A spate of recent studies of botox recipients and others suggests that our emotions are reinforced--perhaps even driven--by their corresponding facial expressions.Charles Darwin first posed the idea that emotional responses influence our feelings in 1872. “The free expression by outward signs of an emotion intensifies it,” he wrote. The esteemed 19th-century psychologist William James went so far as to assert that if a person does not express an emotion, he has not felt it at all. Although few scientists would agree with such a statement today, there is evidence that emotions involve more than just the brain. The face, in particular, appear...

The PREEMPT 1 and 2 studies were conducted at centers in North America and Europe, and included 1,384 patients (average age 41 years). Each trial consisted of a 4-week baseline period, during which patients kept a headache diary, followed by 24 weeks of treatment during which patients received two injection cycles of either placebo or 155 U onabotulinumtoxinA (Botox), which has not been approved for migraine by the Food and Drug Administration. From 24 to 56 weeks, there was an open-label trial consisting of three injection cycles of the study drug, Dr. David W. Dodick of the Mayo Clinic Arizona, Phoenix, reported at the International Headache C...

Botulinum toxin type A in the treatment of painful adductor muscle contracture after total hip arthroplasty.

Authors: Santamato A, Ranieri M, Panza F, Solfrizzi V, Frisardi V, Lapenna LM, Moretti B, Fiore P Painful adductor muscle contracture is an important cause of failure during rehabilitation following total hip arthroplasty (THA). Adductor muscle contracture may be caused by postoperative muscle retractions, adhesive capsulitis, postoperative leg-length inequalities caused by implant failure, or preexisting hip pathologies. A 34-year-old woman experienced a persistent painful contracture into the left adductor magnus muscle after THA. She had no leg-length inequalities and, according to the Medical Research Council scale (grades 0-5), muscle strength of the quadriceps was 5/5 for the right side and 3/5 for the left. The degree of functionality according to the Harris hip score (HHS) was ...

Is active sweating during heat acclimation required for improvements in peripheral sweat gland function?

We investigated whether the eccrine sweat glands must actively produce sweat during heat acclimation if they are to adapt and increase their capacity to sweat. Eight volunteers received intradermal injections of BOTOX, to prevent neural stimulation and sweat production of the sweat glands during heat acclimation, and saline injections as a control in the contralateral forearm. Subjects performed 90 min of moderate-intensity exercise in the heat (35°C, 40% relative humidity) on 10 consecutive days. Heat acclimation decreased end-exercise heart rate (156 ± 22 vs. 138 ± 17 beats/min; P = 0.0001) and rectal temperature (38.2 ± 0.3 vs. 37.9 ± 0.3°C; P = 0.0003) and increased whole body sweat rate (0.70 ± 0.29 vs. 1.06 ± 0.50 l/h; P = 0.030).   Current studies on myofascial pain syndrome

Abstract  Recent studies have clarified the nature of myofascial trigger points (MTrPs). In an MTrP region, multiple hyperirritable loci can be found. The sensory components of the MTrP locus are sensitized nociceptors that are responsible for pain, referred pain, and local twitch responses. The motor components are dysfunctional endplates that are responsible for taut band formation as a result of excessive acetylcholine (ACh) leakage. The concentrations of pain- and inflammation-related substances are increased in the MTrP region. It has been hypothesized that excessive ACh release, sarcomere shortening, and release of sensitizing substances are three essential features that relate to one another in a positive feedback cycle. This MTrP circuit is the connection among spin...
