Source: Am J Cardiology Area: News A retrospective analysis of three trials studying beta-blockade to prevent cardiovascular events in at-risk patients undergoing non-cardiac surgery suggests that the regimen studied did not significantly increase stroke risk.   Peri-operative beta-blockade has been widely used in patients at risk of cardiovascular events who are undergoing non-cardiac surgery, as evidence suggests that it reduces the risk of serious cardiac events. A large controlled trial, however (the POISE trial; Lancet 2008; 371: 1839-47), found that the regimen used in that study, metoprolol m/r 200mg daily, was associated with an increased risk of stroke.   Atrial tachycardia with 1:1 atrioventricular conduction precipitated by propafenone

A 58-year-old man presented to the emergency department with sudden onset rapid palpitations and significant presyncope while walking on the flat. The previous day he had undergone DC cardioversion for atrial fibrillation (AF) which had been initially successful. However, 6 h after cardioversion he was aware of intermittently raised but regular heart rates. On arrival at the emergency department (ED) he was well with no haemodynamic compromise. The ECG showed an atrial tachycardia instead of AF. Medications consisted of propafenone 300 mg twice daily, bisoprolol 5 mg at night and warfain. Bisoprolol was increased to 5 mg twice daily and he was discharged with a plan for outpatient ablation. Chronic β1-adrenoceptor signaling occurs in a failing heart. The use of β-adrenoceptor blockers is beneficial in management of humans with heart failure. Second- and third-generation β-adrenoceptor blockers effects include improved systolic function, prevention of worsening systolic function, and reduced remodeling. Both β1-adrenoceptor-selective agents and third-generation pan-adrenoceptor blocking compounds are effective in prolonging survival in humans with chronic heart failure. CONCLUSIONS: Nebivolol seems better than bisoprolol in CHF by decreasing apoptosis and cytokines induced muscle wastage, preventing fibers shift and protein oxidation. Nebivolol by stimulating NO generation may have prevented protein oxidation. It could be speculated that ROS release, pro-inflammatory cytokines production and NF-kappa-B activation may play a key role.   Hemodynamic evaluation of bisoprolol after coronary artery surgery in patients with altered left ventricular function

Summary  Bisoprolol is a new cardioselective beta1 adrenergic blocking agent without intrinsic sympathomimetic activity but with minimal effects on myocardial contractility. Bisoprolol was compared to propranolol in 24 patients after cardiac surgery for coronary artery bypass graft (CABG). Each patient had been treated preoperatively with beta-blocking agents and had a cineangiographic left ventricular ejection fraction between 35% and 55%.   Unusual presentation of primary toxoplasmosis infection in a kidney-transplant patient complicated by an acute left-ventricular failure.

This report describes this complication. The patient presented with high-grade fever, haemolytic anaemia and haemophagocytic-syndrome-related pancytopaenia. Toxoplasma gondii diagnosis was ascertained by blood and bone-marrow PCR assays. After 6 weeks with Clindamycin plus pyrimethamine therapies and despite negativation of T. gondii blood PCR assay, the patient developed left-ventricular failure. After adding sulfamethoxazole/ trimethoprim, ramipril, digoxine, bisoprolol and spironolactone, he progressively recovered. Anti-T. gondii therapy was continued for 6 months. Four years later he received a third kidney allograft: at that time anti-T. gondii antibodies had become negative.   The efficacy of cibenzoline for reducing the left ventricular pressure gradient of hypertrophic obstructive cardiomyopathy: A case report

Abstract  A 46 year-old man with hypertrophic obstructive cardiomyopathy (HOCM) was referred to this clinic complaining of shortness of breath. His symptoms remained even after he had been treated with bisoprolol, lisinopril, and verapamil. We started treating him with cibenzoline, instead of verapamil. Continuous-wave Doppler examination revealed a marked decrease from 95.9 mm Hg to 8.8 mmHg in the left ventricular outflow tract pressure gradient after administration of cibenzoline, and the symptom disappeared. The electrical axis of the electrocardiogram deviated from +53 to +95 degrees, and the QRS interval was increased from 0.095 to 0.114 seconds.
