  Childhood course of renal insufficiency in a family with a uromodulin gene mutation

This report provides a long-term characterization of renal functional decline in three children from one family with a novel UMOD mutation (c.891T>G, p.C297W) who received allopurinol and a low protein diet. Introduction: Hydrogen sulfide (HS) has been shown by our lab, and others, to significantly mitigate the damage caused by ischemia-reperfusion injury (IRI), but the mechanism of action remains poorly understood. As a partial inhibitor of oxidative phosphorylation (by reversible inhibition of cytochrome c oxidase), HS reduces ATP synthesis with a resultant increase in the activity of xanthine oxidase (XO) and the generation of reactive oxygen species (ROS). We hypothesize that that the low levels of ROS result in the upregulation of natural antioxidant and antiapoptotic pathways, thereby conferring protection against a subsequent profound ischemic event. Granuloma annulare (GA) is a rare disease characterized by granulomatous inflammation of the dermis. A variant form of the disease, disseminated granuloma annulare (DGA), can be observed in about 15% of affected patients. Localized GA is likely to resolve spontaneously within months or a few years, whereas DGA can persist for decades. Various therapies have been suggested in these cases, but none of them has been demonstrated to be consistently efficacious. Urate concentration in serum and urine may provide an initial indication of XDH deficiency before high-performance liquid chromatography (HPLC) analysis is performed. The key to identifying the disorder is a greater awareness of XDH deficiency amongst primary care physicians, nephrologists, and urologists, but also rheumatologists.   Visceral leishmaniasis in a renal transplant recipient treated with allopurinol.

We report a case of visceral leishmaniasis in a 49-year-old male renal transplant recipient, a resident of the western part of Tunisia, which is an endemic zone for the disease. Just before and after the transplantation, the patient resided in Tunis, which is non-endemic for leishmaniasis. Visceral leishmaniasis occurred eight years after renal transplantation, and the clinical picture was characterized by fever and pancytopenia. Leish-maniae were detected by bone marrow aspiration. This study showed that in an in vitro bioassay that Tahitian Noni® Juice (TNJ) inhibited xanthine oxidase (XO) concentration dependently.
