After nasal administration, fexofenadine.HCl in rabbit pl... Conclusions Clearly, the diagnosis and management of DCIS is highly complex with many unanswered questions, including the fundamental natural history of untreated disease. Because of the noninvasive nature of DCIS, coupled with its favorable prognosis, strong consideration should be given to elimination of the use of the anxiety-producing term "carcinoma" from the description of DCIS. The outcomes in women treated with available therapies are excellent. Thus, the primary question for future research must focus on the accurate identification of patient subsets diagnosed with DCIS, including those persons who may be managed with less therapeutic intervention without sacrificing the excellent outcomes presently achieved. Essential in this quest will be the development and validation of accura... From the middle of the media hall in the Bella centre, where the negotiations have been taking place, the Guardian's environment editor, John Vidal, US environment correspondent Suzanne Goldenberg and Asia environment correspondent, Jonathan Watts , analyse the deal made by world leaders. We look at Barack Obama's influence as well as his disappointing speech. We ask whether it was China that held up the talks. Political correspondent Allegra Stratton, who has been travelling with the Downing Street team, tells us how Gordon Brown helped shape the negotiations. Four major NGOs give their reaction to the deal. We take a look at some of the lighter aspects of the final day in Copenhagen including the s... Abstract The clinical course of CLL is highly variable, and survival from the time of diagnosis of CLL can range from months to decades. Novel biological markers such as IgVH mutation, CD38, and ZAP-70 expression have shown to offer important prognostic informations. Few reports deal with the sCD138 levels and bad prognostic factors in patients with CLL, and contrasting data are reported in literature. In our study, we evaluated the serum level of sCD138 in patients with B-CLL and its relationship with other prognostic markers. There was a significant association between advanced Rai stage and serum sCD138 levels in CLL subjects. Patients with Rai stage III-IV had significantly higher levels of sCD138 with respect to controls (48.85 ± 34 ng/ml vs. 31.1 ... Authors: La Mela I, Latagliata EC, Patrono E, Puglisi-Allegra S, Ventura R A major problem in the dietary treatment of disorders associated with excessive eating, such as obesity, is the high rate of relapse into maladaptive eating habits after withdrawal from consumption of palatable, energy-dense food. As olfaction has a major role in appetite and eating behavior, in this study we used a reinstatement model based on conditioned place preference to investigate the ability of olfactory priming to reinstate extinguished chocolate-induced conditioned place preference in sated mice. We found that olfactory priming, which was ineffective in inducing conditioned place preference in the control group, reactivated place preference following the extinction procedure in the experimental group. ... Abstract The present study has been designed to investigate the role of opioid receptors, mast cells, and histamine receptors (H1 subtype) in the seizurogenic effect of tramadol on pentylenetetrazole-treated mice. A single injection of pentylenetetrazole (80 mg kg−1) was used to elicit seizure activity in mice. Seizures were assessed in terms of the time latency of the onset of Straub-like tail, onset of jerky movements of whole body, convulsions, and death. Tramadol administration (50 mg kg −1) caused a marked increase in seizurogenic activity of pentylenetetrazole as measured in terms of a significant decrease in the time latency of the onset of Straub-like tail, jerky movements of whole body, convulsions, and death. Moreover, prior administration o... (Source: Molecular Pharmaceutics) Impact of OATP transporters on pharmacokinetics Membrane transporters are now recognized as important determinants of the transmembrane passage of drugs. Organic anion transporting polypeptides (OATP) form a family of influx transporters expressed in various tissues important for pharmacokinetics. Of the 11 human OATP transporters, OATP1B1, OATP1B3 and OATP2B1 are expressed on the sinusoidal membrane of hepatocytes and can facilitate the liver uptake of their substrate drugs. OATP1A2 is expressed on the luminal membrane of small intestinal enterocytes and at the blood-brain barrier, potentially mediating drug transport at these sites. Several clinically used drugs have been identified as substrates of OATP transporters (e.g. many statins are substrates of OATP1B1). Some drugs may inhibit OATP transporters (e.g. cyclosporine) causing pha... The aim of the present study was to characterize the cellular uptake of levofloxacin using another human intestinal cell line, LS180. Levofloxacin uptake in LS180 cells was temperature-dependent and optimal at neutral pH, but was Na+-independent. The rank order of inhibitory effects of the four compounds on [14C] levofloxacin uptake in LS180 cells was nicotine>enalapril>L-carnitine>fexofenadine, which is consistent with that in Caco-2 cells. The mRNA levels of OATP1A2, 1B1, 1B3 and 2B1 in LS180 cells were markedly different from those in Caco-2 cells, and OATP substrates/inhibitors had no systematic effect on the levofloxacin uptake. Authors: Allegra A, Coppolino G, Bolignano D, Giacobbe MS, Alonci A, D'Angelo A, Bellomo G, Teti D, Loddo S, Musolino C, Buemi M Bone marrow-derived, CD34+ progenitor cells have been shown to promote the repair of damaged tissues, offering promise for the treatment of hereditary and acquired human diseases. These cells in fact differentiate into endothelia, hematopoietic cells and possibly neurons, fibroblasts and muscle. CD34+ and AC133+ progenitor cells may participate in neovascularization by differentiating into endothelial cells. Compared to the control group given fluvastatin alone, the concurrent use of fexofenadine (10 or 20mg/kg) prior to the oral administration of fluvastatin (5mg/kg) decreased the systemic exposure of fluvastatin by 17-51% in rats. Consequently, the bioavailability of oral fluvastatin was significantly lower (p<0.05) in the presence of fexofenadine compared to that from the control group. Furthermore, the intravenous pharmacokinetics of fluvastatin (2mg/kg) was significantly altered by the pretreatment with fexofenadine (20mg/kg, p.o.). The plasma clearance of fluvastatin was reduced by 44% in the presence of fexofenadine. The effect of fluvastatin on the pharmacokinetics of ... This puts the spice in that magic group of foods with health-giving properties - along with garlic, ginger and fish oils.Like ginger, turmeric is a rhizome, a horizontal stem of a plant that grows underground. Generally speaking, it is prized more for its colour than its flavour: turmeric is the oldest natural food dye in the world, and is now seen in ingredients lists as E100. In my opinion, however, this downgrading from flavour to colour is largely due to the spice we get in Britain being old and dull by the time it reaches us. In Zanz... Phase III C-08 Study Of Avastin In Early-Stage Colon Cancer Did Not Meet Primary Endpoint Genentech, Inc. announced that a Phase III study of Avastin ?? (bevacizumab) plus chemotherapy following surgery in patients with early-stage (adjuvant) colon cancer (NSABP C-08) did not meet its primary endpoint of reducing the risk of cancer returning (improvement in disease-free survival). Results are from a planned final analysis of the study. This is the first trial of Avastin in early-stage cancer and results do not affect approved indications in advanced (metastatic) disease. Safety findings were consistent with those presented from this study at the 2008 American Society of Clinical Oncology (ASCO) annual meeting (Allegra et al.). Data from the trial have been submitted for presentation at the upcoming ASCO annual meeting, May 29... (Source: Cancercompass News: Colorectal Cancer) Conclusions: In the studies reviewed, desloratadine, fexofenadine, and levocetirizine were effective in relieving the nasal congestion associated with AR compared with placebo. This effect began as early as day 2 and was consistent and progressive throughout treatment. Desloratadine, fexofenadine, and levocetirizine are appropriate options for the treatment of nasal congestion in patients with AR. PMID: 19539095 [PubMed - in process] (Source: Clinical Therapeutics) To study the effect of fexofenadine on extensive alopecia areata (AA), we evaluated retrospectively 121 patients with AA having alopecia in more than 50% of the scalp and followed them for at least 6 months. Patients were treated by immunotherapy using diphenylcyclopropenone or squaric acid dibutylester with or without oral fexofenadine. The regrowth score was estimated as decrease of Severity of Alopecia Tool (SALT) score. In AA with atopic background (atopic AA), the mean regrowth score of the fexofenadine group was 1.333 (n = 33) and that of the control 0.471 (n = 34). The fexofenadine group showed significantly better regrowth than control by Mann[ndash]Whitney's U-test (P = 0.00213). In non-atopic AA, the mean regrowth score of the fexofenadine group was 1.303 (n = 33) and that of the... Conclusions: Nelfinavir induced methadone clearance by increasing renal clearance, and more so by stereoselectively increasing hepatic metabolism, extraction and clearance. Induction occurred despite 50% inhibition of hepatic CYP3A4/5 activity and more than 75% inhibition of first-pass CYP3A4/5 activity, suggesting little or no role for CYP3A in clinical methadone disposition. The occurrence of antihistamines in sewage waters and in recipient rivers. CONCLUSIONS: Antihistamines are poorly degraded/eliminated under the biological treatment processes applied in the wastewater treatment plants and, consequently, they are continuously being discharged along with other drugs to the aquatic environment. RECOMMENDATIONS AND PERSPECTIVES: As a huge quantity and variety of drugs and their metabolites are continuously discharged to rivers and the sea, the compounds should be considered as contaminants that may possess risks to the aquatic ecosystem. Further studies are urgently needed on the environmental fate of the antihistamines and other pharmaceuticals in the aquatic environment. The currently used histamine antagonists are all inverse agonists that bind and inactivate histamine H1 receptors. Second- generation antihistamines are much more H1-receptor selective with less central nervous system penetration than first-generation agents. Antihistamines typically are more effective in seasonal than perennial AR and do not demonstrate significant relief of nasal congestion. The recent availability of some second-generation antihistamines as over-the-counter products clearly places them as the preferred first-line treatment for mild to moderate AR based on safety when compared with first-generation over-the-counter antihistamines. How Do You Treat Chronic Urticaria? Discussion Chronic urticaria is defined as wheals that occur at least twice weekly for more than 6 weeks. Because this is broad, some people add that the wheals must be present for more than 1 hour (which distinguishes chronic urticaria from dermatographism) and less than 24-36 hours (which distinguishes it from urticaria-vasculitis). The lesions can be 'just' irritating or very painful and even indurated. Outbreaks can occur for weeks-years and affects up to 3 % of the population. Generally there are no systemic problems but some patients have greatly diminished quality of life that is equivalent to people with severe coronary artery disease. Chronic urticaria can be caused by a specific physical stimulus or often is idiopathic. Up to 1/3 of idiopathic cases appear to be autoimmune relate...
