MedWorm: AleveNanoprodrugs of NSAIDs Inhibit the Growth of U87-MG Glioma Cells In this study, we prepared six nanometer-sized prodrugs (nanoprodrugs) of NSAIDs, ibuprofen, indomethacin, and naproxen through the spontaneous emulsification mechanism using monomeric and dimeric derivatives of the NSAIDs. We evaluated their effect on the proliferation of U87-MG glioma cells by cell counting, WST-1 cell proliferation reagent, and propidium iodide incorporation. The two ibuprofen nanoprodrugs inhibited the cell growth more potently than the indomethacin nanoprodrugs, whereas the naproxen nanoprodrugs did not show any significant effect. Remarkably, ibuprofen did not show any effect at an equimolar concentration. Approximately, 4.4% of the ibuprofen nanoprodrugs was found in the cell, whereas no ibuprofen could be detected suggesting that the superior effect of the nano... Authors: Takashima-Hirano M, Shukuri M, Takashima T, Goto M, Wada Y, Watanabe Y, Onoe H, Doi H, Suzuki M Cyclooxygenase (COX) is a critical enzyme in prostaglandin biosynthesis that modulates a wide range of biological functions, such as pain, fever, and so on. To perform in vivo COX imaging by positron emission tomography (PET), we developed a method to incorporate (11)C radionuclide into various 2-arylpropionic acids that have a common methylated structure, particularly among nonsteroidal anti-inflammatory drugs (NSAIDs). Thus, we developed a novel (11)C-radiolabeling methodology based on rapid C-[(11)C]methylation by the reaction of [(11)C]CH(3)I with enolate intermediates generated from the corresponding esters under basic conditions. One-pot hydrolysis of the above [(11)C]methylat... Updated Date: Mar 5, 2010 EST (Source: DailyMed Drug Label Updates for the last seven days (since May 20, 2007 EST)) Meta-Analysis of the Efficacy and Safety of Naproxen Sodium in the Acute Treatment of Migraine Conclusions.[mdash] The available evidence suggests that naproxen sodium is more effective but may cause more adverse events than placebo in the acute treatment of moderate to severe migraine. It is effective in reducing headache intensity, rendering pain-free at 2 hours and improving migraine-associated symptoms. However, its effectiveness relative to other active comparators needs to be better defined by appropriate head-to-head clinical trials. (Headache 2010;[bull][bull]:[bull][bull]-[bull][bull]) (Source: Headache: The Journal of Head and Face Pain) Authors: Fiala M, Frautschy SA Alzheimer disease (AD) is accompanied by an activation of the innate immune system, and many epidemiological studies have shown reduced risk for dementia or AD associated with chronic consumption of non-steroidal anti-inflammatory drugs (NSAIDS). These observations led to animal model studies to test the hypothesis that NSAIDs can be disease-modifying for some aspects of AD pathogenesis. NSAIDS cannot only suppress inflammatory targets, which could contribute to neuroprotection, they also slow amyloid deposition by mechanisms that remain unclear. Several large clinical trials with NSAID therapies with AD patients have failed and cyclooxygenase-2 does not appear to be a useful target for disease modifying therapy. However, there may be apolipoprotein E E4 ... Risk of upper gastrointestinal bleeding among NSAIDs: systematic review of observational studies Source: Arthr Rheum Area: News The risk of risk of upper gastrointestinal bleeding (UGIB) among NSAID users (traditional NSAIDs and coxibs) has been evaluated in a systematic review of observational studies published between 2000 and 2008. The review included 18 original studies initially, 9 of which were excluded for a number of reasons, leaving 9 for the meta-analysis. The following findings were reported: . The relative risk (RR) of UGIB for traditional NSAIDs was 4.50 (3.82 to 5.31) and for coxibs 1.88 (0.96 to 3.71). . Lower RRs than that reported overall were observed for ibuprofen 2.6 (2.17 to 3.33), rofecoxib 2.12 (1.5 to 2.84), aceclofenac 1.44 (0.65 to 3.2) and celecoxib 1.42 (0.85 to 2.37). . Naproxen: Generalised bullous fixed drug eruption: case report (Source: Reactions) Conclusions: Initiating treatment after OH-BBN or when bladder lesions already existed showed naproxen and Iressa to be effective in preventing formation of large cancers. Low dose aspirin and resveratrol were ineffective. In protocol 3, in which microscopic lesions were the end point, only Iressa was effective. Thus, an established cancer end point appears preferable. Naproxen, which has an excellent cardiovascular profile, or epidermal growth factor receptor inhibitors may be effective in an adjuvant setting. (Source: The Journal of Urology) Conclusion This Icelandic nationwide registry-based study amounting to 163,406 patient-years showed increased risk of cardiovascular events, i.e. cerebral infarction, myocardial infarction and unstable angina pectoris, among rofecoxib and naproxen users in comparison to diclofenac users. Henry Black, MD, adds his knowledge to the results of a study comparing naproxcinod to naproxen and whether it means that we will finally be able to relieve chronic pain safely. Medscape Cardiology (Source: Medscape Today Headlines) Authors: Bhise KS, Dhumal RS, Shailesh B, Paradkar AR, Kadam SS The purpose of this research was to address the utility of rheological study in understanding the influence of oppositely charged polymers on release of naproxen sodium encapsulated in chitosan particles. The interaction between oppositely charged kappa-carrageenan (kappa-Ca) and chitosan leads to relatively higher gel strength, which is proportional to the ability to retard the drug release at acidic pH. The oscillatory tests within the linear viscoelastic range where the stress is proportional to the applied strain were performed on the hydrated sample matrices containing chitosan-naproxen sodium spray-dried complexes and k-Ca or hydroxypropyl methylcellulose (HPMC) in various ratios. For this purpose, regenerating tissue from animals in contact with the drug was submitted to histochemical and ultrastructural analysis in comparison to tissue from animals under normal conditions, i.e., not in contact with the drug in question. Actinotrichia regeneration was similar in both animals, indicating that naproxen, at the dose used in the present study, did not interfere with actinotrichia synthesis during the regenerative process of the tail fin. This could be because naproxen did not influence the expression of the genes required for the regeneration process, such as the Sonic hedgeh... The patient's serum potassium concentration was greater than 8 mEq/L and his serum creatinine concentration was 7 mg/dl. Findings on electrocardiography were abnormal. Of note, his drug therapy included lisinopril and naproxen. After treatment for hyperkalemia, the patient's symptoms resolved; however, he was admitted for further workup for renal failure. The patient was discharged after approximately 1 week with a diagnosis of end-stage renal disease. Use of the Naranjo adverse drug reaction probability scale indicated a probable relationship (score of 5) between the patient's paralysis and hyperkalemia. Although hyperkal... Quantitative estimation of naproxen in tablets using ibuprofen sodium as hydrotropic agent Maheshwari R K, Wanare G, Chahar Nitika, Joshi Prajakta, Nayak NishiIndian Journal of Pharmaceutical Sciences 2009 71(3):335-337In the present investigation 0.5 M ibuprofen sodium solution has been used as hydrotropic solubilizing agent for naproxen, a poorly water-soluble drug and in it there was more than 350 fold enhancement in the solubility of naproxen as compared to the solubility in distilled water. Therefore, this hydrotropic solution was employed to extract out the drug from its tablet dosage form for quantitative estimation by titrimetry. The naproxen has been successfully analyzed in tablets. The results of analysis obtained by proposed method compared well with those by corresponding British pharmacopoeial method involving the use of methanol. The proposed method was also valid... (Source: Pain Medicine) Traditional nonsteroidal anti-inflammatory drugs are associated with the destabilization of blood pressure (BP) control, particularly in hypertensive patients treated with blockers of the renin-angiotensin system. To assess the potential impact of nitric oxide donation, the effects of naproxcinod with naproxen and placebo on changes in BP were compared in a randomized clinical trial of 916 patients with osteoarthritis after 13 weeks of therapy. In addition, the effects of naproxcinod versus naproxen and placebo on systolic BP in patients with hypertension treated with renin-angiotensin system blockers were evaluated. A conditional sequential sampling procedure for drug safety surveillance We propose a practical group sequential method, a conditional sequential sampling procedure, to test if a drug of interest (D) leads to an elevated risk for an adverse event E compared with a comparison drug C. The method is designed for prospective drug safety surveillance studies, in which, for each considered drug, a summary table with the exposed person-times and the associated numbers of adverse events summed by strata defined by several potential confounders, is collected and updated periodically using the health plans' administrative claims data. This new approach can be applied to test for elevated relative risk whenever the data are updated. Our approach adjusts for multiple testing to preserve the overall type I error with any specified [alpha]-spending function. Furthermore, it ...
